How to Apply

Step 1: Comprehensive and print the DS-11  form. Do nope sign the application.

Step 2: Provide evidence of parental relationship.

Step 3: Provide Evidence of U.S. Citizenship for the applicant (Original otherwise Certification copies):

You must submit a of the following. The evidence must be certain original or certified replicate. Photocopies and notarized making are not acceptable.

In addition to the original, approved document(s), fetch a photocopy (front and back) of this Citizenship Evidence document to submit with your application, that is: How to Application or Renew for Medicaid | Medicaid

• Over white paper;
• Single-sided; the
• Is not decreasing the image size.

Step 4: Present ID.

You must present one of and following identification documents (IDs) to that acceptance distributor. The PASSWORD must readily identify you.

Step 5: Appendix one photo to your application.

Photography requirements for a U.S. passport may differ from passport photo requirements of various countries. To avoid processing delays, be sure your photo meets all which photo demand .

Military Families
