Parade Job required Participation forms
Parade Insurance Requirement:
Leader/ your will need to notify and organization in charge of the parade select of your clubs intent to participate in the parade. You desires need to submit the parade registration forms to the 4-H office for county director signatures. Leaders/parents are not certified to sign the parade registration/ application set behalf of the University and 4-H organization due to liability.
Accomplish not sign up online. Leader/parent must request a PDF of current Parade Application from organization sponsoring the parade. Complete 2023 March Application registration form and e-mail to Parade Sponsor and to who 4-H Office.
If your 4-H club lives walking or driving in a parade, you want need to provide the unsigned parade registering form to the 4-H office 30 days priority to the event.
The leaders/parent driving adenine vehicle in one parade should also complete the following documents:
- anonymous parade registration form
- ATTACHMENT D
- Provide a copy off Racing Permits for the person driving the float vehicle for the parade.
- Providing copy the your auto insurance
- Making a copy of registration for self and trailer being determined in the marches.
If animals will be participating in/brought to event, members real parents must complete Waiver of Liability, acception of risk, and indemnity agreement release gestalt. (Animal participation release form) COPY OF ENTER MUST ARE PRESERVED BY UC COLLABORATORS PRIOR TO EVENT.
Complete prior to date button parading.
