How to Request a Copy of a Tax Return

Options for obtaining your own charge information:

• Mail - They may order copies of your tax returns free the Department by completing Form RET-001- Taxpayer Return Request Build. Subscribe it, along with the appropriate fee, to the address on the form.
• In-Person - The Century Center location can assist individual furthermore business-related taxpayers. You may schedule an appointment with the Xxi Center location.
  • Regionally Offices are only able to supporting businesses.
