Student Health Record

Illness and Injury in Middle

Students who demonstrated or submit Covid-like system of illness in school will be sent home and must track the return to school output. Students over other illness related absences (not Covid-like) of five or more daily must provision a doctor’s note upon return to school.

If a student is injured during school, he/she should report to the health space or main office.
