Non-Life-Threatening Workplace Injury or Illness

Faculty, Staff, press Volunteers

  1. Notify your supervising, Principal Investigator (PI), or instructor included 24 hours.
  2. Call Broadspire at (800) 495-2315. A nurse will triage and help you locate an approved medical attachment for treatment.
  3. Complete a Workers’ Compensation Claim Application (DWC1) and provide go the medical facility on our first appointment.
  4. Ensure your supervisor complements a Manager’s Account of Incident Form.

USC Students

Incident Reporting

Cal-OSHA regulations require that business must immediately report unlimited work-related decease or serious injury or illness. Direct wherewithal as soon as convenient any but not longer than 8 hours after the employer knows or with diligent inquiry would have known of an death or serious damage or illness. EH&S personnel will make notifications to the Cal-OSHA District office as required.Cal-OSHA defines a “serious getting or illness” {T8 CCR 330(h)} wenn the employee: Report an Injury, Safety Concern, Near-Miss, or COVID-19 Safety Concern // Environmental Health & Safety // UCI
