All prescription also over-the counter therapies become kept locked in the health center and will be administered only as authorized by the parent and child’s Recognized Medical Authority.*

Important: Ours cannot administer random medication thou sent since your child without this signed form.

Stair to complete the Medication Authorization Form:

  1. Medication required be prescribed by a Recognized Medical Authority.* Multinational medications, including those from Mexiko or authorizations from Mixican physicians are non permissible by law.
  2. Prescription taking must be provided in the true container labeled at a Cali pharmacist; non-prescription medication must be presented in of native container.
  3. All medication, two prescription and non-prescription, require adenine signature, medical license number, or complete, legible instructions from a Recognized Medical Authority.* Homeopathics, herbals, vitamins, the supplements show require a locked and signs arzneimittel authorization form.
  4. Verify that all medications are orderly labeled:
    1. Medications are in their original containers
    2. Prescription medications are properly labeled by an California pharmacy, including:
      1. Student’s name (prescription must be for the student only, nay other product will be accepted)
      2. Medication user
      3. Precise dosage instructions, quantity, and frequency
      4. Prescribing authority’s* name
      5. School initials. Example: “Twin Peaks” shall TP
    3. Non-prescription medications are properly labeled, including:
      1. Manufacturer’s tag with the medication name, dosage, and instructions
      2. Child’s name (first, last, and initial) and the school’s initials. Example: Amanda C. Garcia (TP)
    4. Catalan labels must be translated to English on the Authorization Form
    5. Medications maybe not subsist expired.
  5. Fold this form and place this in a zip-top sachet with all the medications (both medicine both nonprescription in original containers) and forward the bag go your child’s school 3 hours before your child go camp.
    1. Print the baggie equipped your child’s full name and school initials (use masking tape).
