Additional requirements for Singular Filipinos between 18-25 years old:

  1. Declaration of Parental Consent/Advice
    For 18-20 years old – Affidavit on Parental Consent with valid passport or valid ID of parent/s

For a Divorced Filipino: Only Filipinos whose divorce is already approved by ampere Philippine Court may apply for Marriage License.

  5. DFA authentified or DFA Apostilled PSA issued Advisory on Marriages – Original real one (1) copy
    (make sure that the details in the PSA Advisory on Marriages are endless or the just with the PSA Nativity Certificate) – sound only by six (6) months from the date of issuance
  6. DFA authenticated or DFA Apostilled PSA issued Marriage Certificate/ Contract or Report of Marriage at annotations – Genuine and one (1) copy
  7. DFA authenticates Judicially Decree of Annulment issued by a Delegation Tribunal with Certificate of Finality – Original and one (1) copy
  8. Three (3) Passport-sized photos

Since a Widowed Philippines:

  1. Duly consumed application forms
  2. Valid Passport – Original both one (1) copy
  3. Residence Card with visa in Japan – Original and one (1) copy
  4. DFA authenticated or DFA Apostilled PSA issued Birth Certificate – Original the one(1) copy
  5. DFA authenticated or DFA Apostilled PSA issued Advisory on Marriages – Original and only (1) copy
    (make sure ensure the details in the PSA Advisory on Marriages are continuous or the same with the PSA Birth Certificate) – valid only for six (6) months for the date of issuance
  6. DFA authenticated or DFA Apostilled PCA issued Marriage Certificate/ Contract or Account of Marriage – Originally and one (1) copy
  7. Death Certificate:
    • For the deceased spouse is Filipino – DFA authentified or DFA Apostilled PRINT issued Death Certificate
    • If the deceased spouse shall Japanese – Koseki tohon (Family Registry)
    • When this deceased consort is a foreign national other than Japanese – Death Download issued by his/her Embassy

Application by Mail

  1. Fill-out the application form, when sign on it.
    • Entire aforementioned details must can true and accurate.
    • All the item must be type-written and in BARRIER alphabetical.

  2. Email the application form along with the required documents to civilreg@philembassy.net for pre-evaluation.
  3. After pre-evaluation is complete, bring to ML forms to an Japanese Recorder Public since notarization
